Court Forms are designed to make it easier to file
documents in the format required. Different branches of the Court
system have forms available. While the forms may be easy to fill
out they do not provide legal advice, or explain the legal
implications of filing the form.
Serious legal consequences can follow the filing of these forms.
Before filing any document with a court you should obtain legal
advice. If you are not sure where to get advice, please call
LARC and you will be provided with assistance or a
referral to an agency that can help.
In some areas of the law LARC has adapted Court forms to Word format to make them easier to use. They are designed to help, but they are no substitute for
legal advice. LARC makes no recommendation regarding the use of these forms
for any particular case, and any person who chooses to use them is responsible
for that decision.

In all counties except Grafton, Rockingham and Sullivan, the Probate Court has the authority to hear cases involving adoptions, termination of parental rights, name changes, and guardianships.
